What happens when care becomes more than a routine?

For Shawn, it meant having someone who knew him - not just his care needs, but his routines, preferences, personality, and the little things that make daily life work.

When Shawn met Rabiaa, “we just clicked.”

Over the past two years, that connection has grown into something powerful: a partnership built on trust, patience, respect, and a shared commitment to finding solutions together.

From keeping track of appointments to supporting Shawn’s independence, their story shows how consistency can transform care.

Read Shawn and Rabiaa’s full story:

Shawn knew that consistency mattered. Before working with Rabiaa, he had different caregivers on different days. Each person came to provide support, but he knew that having someone familiar and tr…

For the past eight years, Bernadette has done more than help older adults and people with complex needs with everyday tasks - she's built relationships that make people feel safe, seen, and supported.

She knows that the strongest care grows through trust, familiar faces, and showing up day after day. Those connections don't just help people remain at home - they bring comfort to clients, peace of mind to families, and remind us that care is deeply personal.

As Bernadette says, "Everyone needs help. It's about helping people."

Read her story and discover how lasting relationships are at the heart of great care:

Every day, Bernadette helps older adults and people with disabilities with the routines that make it possible to live safely at home. She shops for groceries, pays bills, coordinates doctor’s…

Big news 🎉

Homebridge Inc. is expanding into Alameda County through a new partnership with Alameda Alliance for Health – marking Homebridge’s first direct care expansion outside of San Francisco.

“This expansion reflects where health care is heading — toward more integrated, community-based support,” said Bazil Fonseca, Senior Director of Growth & Innovation at Homebridge.

Under CalAIM, we’re bringing community-based support directly into housing sites to help Medi-Cal members access care coordination and essential services - because real impact happens when care meets people where they are 💙

The expansion strengthens access to community-based care for Alameda County residents and aligns with CalAIM initiatives to improve whole-person health outcomes. Alameda, CA – Homebridge anno…

Last week, we had the opportunity to welcome Housing Works of California Executive Director Celina Alvarez and Michael De La Torre for a thoughtful conversation about the shared challenges and opportunities facing mission-driven organizations serving vulnerable communities.

Conversations like these are an important reminder that organizations across California are often navigating many of the same experiences. By collaborating, sharing expertise, and learning from one another, we can continue to advance innovative and sustainable solutions that create greater stability and opportunity for the people we serve.

We were honored to participate in the Institute on Aging AAPI Leadership Panel, where leaders across healthcare, home care, and affordable housing came together to share their experiences, perspectives, and commitment to community impact.

The conversation explored personal journeys, leadership lessons, and the importance of leading with service, empowerment, and a dedication to uplifting others and strengthening communities.

CEO Min Chang reflected on what makes her work especially meaningful: “The opportunity to directly touch the community and make a difference in people’s lives - that’s so worthy to celebrate.”

We’re grateful to be part of conversations that amplify voices and advance more inclusive, community-centered leadership across the sectors that shape , , and .

Last month, attended the Community Care Initiative (CCI) in San Francisco – joining caregivers, community members, and champions of care in a celebration of connection and community.

The gathering brought together advocates and thought leaders for cross-sector dialogue on the future of caregiving, where a shared truth emerged: caregiving is essential infrastructure for resilient communities.

We’re grateful to our partners at Rework the Bay, San Francisco Foundation, RepresentED Leadership, WISE Health, Bayview Senior Services, SF IHSS Public Authority, and Bay Area Black Nurses Association (BABNA) for fostering meaningful dialogue and collaboration. We look forward to continuing this work in support of caregivers and stronger care ecosystems in San Francisco and beyond.
