2026 Retiree Healthcare Fund Legislative Questionnaire for Spousal
coverage - Fill out form [1]

The Fund is collecting information about spouses who are not currently
covered under the Plan. One legislative proposal being considered would
allow coverage for remarried members' spouses, and we need accurate
information to study the potential cost and impact of this change. To
complete an actuarial study, we are gathering information on the number
of spouses not currently covered and their age groups. If you remarried
after retirement, please complete and submit the survey. Your
participation will help ensure the Fund has the information needed to
fully evaluate this proposal. If you prefer, you may come by or call the
Fund office at 1-866-652-4237 and provide the information directly to a
Benefits Specialist.

The survey will be available starting today, Friday, August 21, 2026,
and will continue until 5:00 p.m. on Wednesday, August 26, 2026, using
one of the following methods.

* Scan the QR code attached or open the link
* Call the Health Fund Office at 1-866-652-4237 for assistance.
* Visit our office, where a Benefits Specialist will be available to
help you complete the survey.

information will be held strictly confidential by the Fund. Anonymized
data (information that has had all personal identifiers permanently
removed), will be provided to the Fund's actuary for purposes of

governing statute._

When "To Hell and Back" premiered in S.A my grandfather was with SAPD. He actually was bodyguard for Audie and was with him the entire time he was in SA and stayed in same room. The uniform Audie wore was my grandfather's they actually had to have it tailored for Audie because my grandfather was so tall.
