We would like to thank you all for allowing CARE (Connecting All Resources to Empower) at New Transitions, Inc. to serve you all over the years. As of July 1, 2020 we will no longer operate the navigation services program in Wicomico County as it will be serviced by another provider in the community.

Please continue to refer individuals to our program or make contact with your need for resources with the understanding we will provide information and/or referral. We again appreciate serving those in the community over the past years as CARE has been a program within New Transitions, Inc.
