What happens when your doctor recommends care, but your insurance company says no?

Attorney Lisa Kantor says denials are not isolated mistakes. Patients can be denied cancer treatment, essential medical equipment, or continued eating disorder care, even when their own providers say it is medically necessary.

Her proposed solution is simple: “The presumption should be coverage.” Approve the doctor’s request first, then require the insurer to prove why it should not be covered.
