Locations in the 19th Ward and Beechwood communities. What Does His Branches Do? We are a faith based 501c3 ministry operating a Federally Qualified Health Center and focusing on medical care and community development.

1. Medical Care - operating a full service Family Practice including women's health, laboratory services, and behavioral health. We have two locations: (and yes, we are accepting

The power of a community health center is…the community.

Since launching in 2021, Center 4 Hope has grown to serve 800+ clients each month (food market, resource navigation, and financial counseling). Their clients and the Greece community have told us what they need most: access to primary care.

Healthier communities happen when we listen, partner, and grow together.

So, we’re excited to announce that we’re partnering with them to expand access to medical and mental health care by launching a NEW LOCATION under the same roof as Center 4 Hope!!

Architectural work has begun with Rozzi Architects and we’re slated to complete construction in order to open in Summer 2027.

Toward longer and healthier lives, one neighbor(hood) at a time!

Envision Thurston Road aims to improve Thurston by bringing in more businesses and by encouraging people who live in the 19th Ward to shop there. It is a three-year project, with 2026 centered on gathering information, building connections, and developing a strategy for improving the corridor. 2027 will be focused on testing ideas from that strategy, and 2028 will be about building the systems the neighborhood needs to continue the work into the future. The work is supported by project management from the PathStone Enterprise Center, consulting by CivicBrand and G. Kellogg & Co, and oversight by the Thurston Corridor Advisory Board.
