Sometimes a relatively small need can become a major crisis.

The Center for Healthy Aging’s Elder Gap Program helps low-income older adults, caregivers, and people with disabilities access short-term support that helps them remain safely in their homes and communities. The program may assist with needs that fall through the cracks when other funding or services aren’t available.

That can include things like:
💙 Short-term support services
💙 Limited respite assistance for caregivers
💙 Help accessing essential items or resources
💙 Support that helps people continue living independently

The program is designed to help fill gaps before a situation becomes an emergency.
