1. You must show the birth certificate listing you as the biological parent or have legal documentation you are the guardian. Stepchildren must be registered by the biological father/mother.
2. You must bring these documents to 921 W. Mayfield Road #102, Arlington, TX 76001 between the hours of 8-12pm and 1-4pm Monday-Thursday.
3. The QR Code is to hold a place for you to sign in with all the information requested on the form.
4. If you need assistance to sign up, you may do so during the times listed above.
December 10th is the last day to register.
