For several years Hospice of Wake County has offered more than “hospice” care and has served patients well beyond “Wake County.” Though we changed our name to Transitions LifeCare, our mission and vision are unchanged and we remain a local, non-profit serving patients regardless of their ability to pay. And we serve more than Wake County. Our service territory extends well beyond Wake County so we help more patients and families in Durham, Johnston, Franklin, Harnett, Wilson, and Nash counties, eastern Chatham County and in the communities of Chapel Hill and Carrboro in Orange County. Rest assured our expertise, compassion and commitment to helping families facing a serious or terminal illness remain the same. We are still a non-profit organization with a local board of directors. We have not been bought by a large, for-profit corporation. Going forward, Transitions LifeCare is our single brand name and our service lines have corresponding names: Transitions HospiceCare, Transitions Kids, Transitions PalliativeCare, and Transitions GriefCare.
