We had an opportunity to review these airway manakins at our National Association of Emergency Medical Technicians TECC course at from decentsimulators. These were very realistic from the weight of the head, the skin to the anatomy using VL, DL, i-gel, etc. Thanks Resuscitation and Medical Preparedness, LLC

🚨 Should videolaryngoscopy be the default in 2026?

We were all trained with direct laryngoscopy as the standard.

But the paradigm is clearly shifting.

📚 European recommendations (ESAIC / EJA, 2025) support:
➡️ Videolaryngoscopy (VL) as a first-line approach
➡️ Not just for difficult airways — but for routine practice

But in daily practice… many still default to direct laryngoscopy.

🔍 Why does this matter?

Because first-pass success is everything:
• ↓ Hypoxia
• ↓ Airway trauma
• ↓ Esophageal intubation

And most importantly:
👉 Many difficult airways are unexpected

🧠 Take-home:
This is no longer about device preference —
it’s about maximizing first-pass success and patient safety

Stabbed in the eye after having a fight over a parking lot!! ⚠️
A 47 year old man has suffered horrific injuries after he was stabbed in the eye during an argument over a parking space at a shopping centre. The shocking incident took place in the eastern Mexican state of Veracruz, after the victim and another driver began fighting over a parking space.
A scan taken at the hospital shows how the knife passed through the left eye and through into the lower half of the right orbit (see the X-ray image that shows the knife embedded in the man's eye).
Miraculously, doctors have successfully removed the knife which had pierced the left eyeball and the lower part of the right eye.
Although the patient lost his left eye in the incident he was in a stable condition the whole time.
What are your thoughts on this?
