MESSAGE FROM VASSAR:

"Northwell Vassar Brothers Medical Center and Northern Dutchess Hospitals have instituted a Post Partum Bracelet Initiative to address frequently missed complications in the immediate postpartum window. On discharge, patients are given these bracelets which include a QR code and are instructed to wear it for 12 weeks. It provides information to the patients as well as a pathway for providers to help address OBGYN specific pathology. The attached documentation will include more details and demonstrate the provider pathway found on the QR code. Please reach out to Dr. Wolf at [email protected] with any questions."
