Community Health Workers serve as a bridge between clinicians, their patients, and their communities. Their unique knowledge and skills can support providers to build a deeper understanding of the communities they serve, and fine-tune their approach to help patients with complex needs improve their health and avoid costly emergency department and hospital visits.

As members of the communities they serve, Community Health Workers are able to build strong, trusting relationships with patients. This allows them to successfully address social determinants of health, reduce health disparities, and improve patients’ experience of care. When fully integrated into primary care environments, Community Health Worker programs have been shown to help improve disease outcomes for patients with asthma, hypertension, diabetes, cancer, tuberculosis, HIV/AIDS, and depression, among other conditions.

Join presenters including Jennine Smart and Yesi Castro from Oregon Community Health Workers Association, Sabrina Ballew from Mercy Flights Inc, and team members from Multnomah County Health Department and CareOregon, as they unpack the integration and value of CHWs in primary care, how relationships with clinical staff make for effective collaboration, as well as the intersection between CHWs and health equity during the pandemic and beyond.
