South Shore Community Partners in Prevention WHAT IS A CHNA? The Massachusetts Department of Public Health (DPH) established CHNAs in 1992.

Community Health Network Areas (CHNAs) are coalitions of public, nonprofit & private sectors working together to build healthier communities through community-based prevention planning & health promotion. Today, CHNAs involve each of the stat's 351 towns & cities through 27 networks. CHNAs play an important role in addressing DPH's statewide public health priorities:
- Eliminating racial & ethnic

health disparities & their social determinants
- Promoting wellness in the home, workplace, school & community
- Preventing & managing chronic disease

CHNAs collaboratively identify local & regional health priorities, design community-based prevention plans & track success in achieving healthier communities. CHNAs develop new health improvement projects as initial projects are completed. ABOUT SOUTH SHORE COMMUNITY PARTNERS IN PREVENTION (CHNA 23)

South Shore Community Partners in Prevention represents the towns of Carver, Duxbury, Halifax, Hanover, Hanson, Kingston, Marshfield, Pembroke, Plymouth, Plympton & Rockland. CHNA 23's goals are to:
- Build collaborations focused on DPH's statewide public health priorities
- Support the assessment & response to unmet community health & wellness needs
- Promote the work of CHNA 23 within the region
- Support & promote the work of individual CHNA members

ℹ️ Plymouth ARISE Grant Applications Due May 1st! ℹ️

The Plymouth Addiction Response and Improvement Strategies Effort (ARISE) FY27 grant application is now open!

Plymouth ARISE is the Town’s coordinated response to the local opioid epidemic that aims to increase community engagement and reduce stigma, while enhancing opioid-related resources and services for Plymouth residents and families.

Each year, Plymouth ARISE awards grant funding to selected applicants that implement innovative projects in our community. 501(c)3 nonprofit organizations and municipal government entities are eligible to apply.

We had a great panel discussion on Autism for our April General meeting. Thank you to the Futures Program in Plymouth for hosting and sharing information about ABA therapy. Thanks also to Dr Catherine Linehan of South Shore Health for discussing the hospital’s new sensory-informed care model, and to the ARC of Plymouth and Upper Cape COD for spreading the word about the Greater Plymouth Autism Services program for adults on the spectrum.

🚨 Big opportunity!

Through its Community-Based Health Initiative (CHI), Beth Israel Deaconess Hospital–Plymouth is investing over $1.6 million between 2026 and 2029 to support programs that address the social determinants of health — the conditions in which people are born, live, work, and play.

Funding opportunities include:
🏠 $803,019.97 focused on housing (1 grant)
🩺 $803,019.97 focused on equitable access to care (up to 3 grants)

These grants are part of the hospital’s Determination of Need (DoN) investment related to the relocation and expansion of hematology-oncology and infusion clinics at Cordage Park.

Our December meeting was hosted by the JPH Organization at Ferry Hill in Marshfield, a nurturing community of wellness, healing and transformation. We learned about their many FREE programs and their volunteer-led activities. Our October Meeting featured a belated presentation of the 2025 Health Literacy Hero award to Dr Janet Wade, host of the Healthy Kingston show on the Local Seen.Janet gave a great speech explaining her inspiration to pursue health education and her ongoing efforts to enhance health literacy in the community. Karen Keane, director of Public Health for the Town of Plymouth, discussed the South Plymouth County Public Health Collaborative, a six-town initiative to use data and proven models to improve health outcomes for all.

Thank you to Rep. Kathy LaNatra for attending and presenting a State House citation to Janet Wade. Looking forward to more great programming this fall!
