CP-C3 Safe and Sound-Community Paramedicine.

C3 "Safe & Sound" Community Paramedicine program is designed to combine pre-hospital medical servcies and at-home healthcare. The Community Paramedicine (CP) program's mission is to follow patients with an extensive health history after being recently discharged from the hospital and to provide wellness visits at their home with the goal of refraining from having to go back to the hospital. This p

rogram isn't limited to hospital discharged patients but also open to patients that request routine wellness visits to continue to live a healthy lifestyle. Our interventions to keep people "safe and sound" include reviewing any issues with medications, scheduling physician appointments, providing home health equipment such as diabetic glucometers and pulse oximetry to COPD patients. "CP C3" is able to connect patients with social services and most importantly, provide quality education regarding the patient's health, a new diagnosis, or a new medication. Our CP teams are able to obtain at-home EKGs and transmit the results as well as vital signs and the summary of our visit to the patent's physician(s). Our mission is to build a relationship with each patient by providing compassion, care, and coordination (C3).
