Peninsula Healthcare Connection Serving vulnerable populations by providing medical, case-management, and community resources. PHC services the north peninsula of the San Francisco Bay area.

Peninsula HealthCare Connection (PHC) is a not-for-profit organization with a focused mission to serve the needs of the homeless and the at-risk of becoming homeless. Medical, psychiatric and intensive case management is provided within a multi-service building complex with three other entities; Downtown Streets Team, InnVision, and the Housing Authority. The organizations operate cooperatively in

partnership to deliver health care, social services and housing. PHC provides services through a collage of community agencies consisting of staff, trainees and volunteers including the Palo Alto Medical Foundation and Stanford University as well as the generosity of many. Our common thread is our mutual commitment to the promotion and maintenance of human dignity, health and safety.

Three years ago today, we opened our doors at ISC and welcomed our first residents!

Since then, there have been plenty of unexpected challenges, changes, and lessons learned, but our team has met every one of them with creativity, and compassion.

In fact, today we celebrate that the majority of residents who moved in when ISC opened, many of whom had experienced chronic homelessness, remain housed three years later.

Ability Link helps aging adults, people with disabilities, and their families navigate aging, disability, and complex care through personalized support from our deeply skilled staff.

We help with:
-Life transitions & hospital discharges
- Managing diagnoses & coordinating care
-Planning for safe, independent living
-Supporting families near and far
-Navigating healthcare, housing & insurance systems
-Caregiver relief & care coordination
-Reducing isolation through community connection and home visits

Our clinic and ISC staff enjoyed spending the evening with residents, community partners, and local first responders. Juneteenth marks the day freedom reached Black Americans who had long been denied it, reminding us that legality and justice are not always the same. It’s a day to honor Black resilience, reflect on the unfinished work toward equity, and recognize that the pursuit of freedom is ongoing.
