Healthy Routes (Bay Area)

Healthy Routes (Bay Area) Healthy Routes works to improve health disparities through community-based projects that seek to improve health outcomes for everyone in our communities.

We are an outreach group that wants to not only gain a deeper understanding of health and needs within our community, but we also want to share with and learn from the people whom we serve. In collaboration with partner organizations, we are committed to help support and sustain programs that effectively work to improve physical, mental, and emotional health by providing resources and/or services to individuals who have limited or no access to the social determinants of health.
