ODDS service equity survey for I/DD community members

Hello,
The Office of Developmental Disabilities Services (ODDS) is doing a community survey. We’re assessing service equity in Oregon’s I/DD service system. Service equity means that every person is treated fairly and has a chance to get services and succeed.
We are working with the Oregon Department of Human Services Office of Equity and Multicultural Services (OEMS) to do the survey. It will help us know how I/DD services are getting delivered and how we can improve. We will use the survey’s results to develop the ODDS Service Equity Plan.
The survey is for:
Individuals who get I/DD services, and
Their family members and guardians.
The survey is private and optional. Your name will not be linked to your answers.
How to take the survey
The survey is online. It will take you about 10‒15 minutes.
Deadline: The survey closes on September 11.
You can take the survey in English, Spanish, Vietnamese, Russian, Simplified Chinese, Somali or Arabic.

At the top of the screen, look for the word “Language” with the word “English” below it.
Click on the word, “English.” A list of options will open.
Click on the language that works best for you.
