Answer: B. The client states understanding of the proposed procedure as well as acceptance of the risks involved and the right to refuse treatment. The client demonstrates competence to sign the informed consent document by appropriate conversation and asking for a clipboard to write on while signing the consent, further correctly identifying the document and necessary
action required. No person with dementia can legally make an informed decision for an invasive procedure, even if the client demonstrates occasional lucid thought and is oriented to self. A minor child cannot legally consent to an invasive procedure unless the child is an emancipated minor.

The client has lucid moments and is oriented to self, but is generally disoriented regarding place and circumstances. There is an order on the chart to obtain informed consent for placement of a central intravenous line.
D. A 14-year-old client brought to the emergency department states, “I can sign the consent for my procedure. My parents are at work and will not be here for a while. They won’t mind me signing the consent.”
