Early detection, intervention and connection to services are critical for optimal outcomes for children with developmental, behavioral or educational concerns, but too often children miss this opportunity. HMG addresses this by building collaboration among community programs; providing a centralized access point for information and referral; promoting surveillance and screenings; and educating and informing the community.
HMG does not provide direct services. Rather, it supports families, child health care, early care, education and human service providers by improving access to existing resources and services for children through age eight.
The core components of the HMG system have been shown to decrease medical costs, build family resilience and protective factors, as well as maximize the efficiency of social service networks by creating a pathway for access to services through a central utilization model. HMG’s four core components are: health care provider outreach, community outreach, central telephone access and data collection.
HMG provides child health care and education provider outreach trainings to educate communities about child development and the importance of early detection and intervention, as well as developmental milestones children should reach in how they play, learn, speak, behave and more.
HMG engages in community outreach to promote the use of HMG and to provide networking opportunities for the community.
HMG utilizes 2-1-1, a telephone access point which links families of young children with sources of support, information and services to address concerns about child health, development, behavior and learning in a child’s first eight years of life. The call is free and confidential.
HMG collects data to understand all aspects of the HMG system, including the identification of gaps and barriers in order to provide more effective services
