Team Member Tuesday: Welcome to the ACCA team Yvette! Yvette joined the San Mateo and Santa Clara team as a Care Navigator in July. She brings over 25 years of Clinical and Administrative experiences from working as a Certified Medical Assistant (CMA) and Phlebotomist. Prior to joining ACCA she worked and cared for those experiencing homelessness or at risk for experiencing homelessness, assisting them with healthcare insurance enrollment as well as addressing their medical, physical, and mental health needs. We are excited to have Yvette join our team and she has already started enrolling individuals in ACCA's Enhanced Care Management program. Team Member Tuesday: We're excited to have Mel Mobini join the ACCA team! She joined ACCA in March and jumped right in to learn the program and support the team with administrative duties. Mel brings her experience in in communications and coordination to her work as an ACCA Care Navigator and chose to join ACCA because she believes in meeting people where they are at - with compassion, patience, and hope. Melika will be serving Enhanced Care Management participants in our Bay Area counties. We are excited to have Mel on our team! Team Member Tuesday: Meet one of our newest team members, Sifany Reyes! She joined the team just a few weeks ago and is already a core part of our care team in Los Angeles, poised to serve both English and Spanish-speaking participants. She brings a business degree and experience working as a paraprofessional in the K-12 education system to her work as an ACCA Care Navigator as well as experience volunteering and organizing community toy drives that has deepened her commitment to making a positive impact in her community. We are excited to have Sifany a member of the team. On Saturday, Our LA Care Team participated in the inaugural grief and end-of-life resource fair hosted by PAUSE in Los Angeles. Regional Director, Trevino Cochran, and Care Navigator, Nikki High (pictured here) hosted a resource table sharing how ACCA's free care navigation services can assist persons with chronic, serious, or advanced illnesses and their caregivers. ACCA also hosted a Caregiver Workshop featuring ACCA's new and improved Workbook for Caregivers (link in bio). More than 50 people attended the resource fair and several attended the workshop. PAUSE is dedicated to supporting people of color through grief and end-of-life with safe, culturally sensitive, and expert-informed resources. Thanks for having us!

This Saturday, November 23rd visit the PAUSE End-of-Life and Grief free resource fair in Los Angeles. Connect with over a dozen providers and experts in grief and end-of-life care, meet community organizations who support individuals in this area, and sign up for more than 12 interactive workshops including ACCA's Confidence for Caregivers! Our LA Care Team will share how ACCA's Workbook for Caregivers can help caregivers organize their care recipients' needs, build their caregiving support network, and take care of their own physical and mental health to make their caregiving journey easier. We hope to see you there!

This last weekend ACCA joined the Sankofa Elders Project Resource Fair in Los Angeles. Trevino Cochran, ACCA's Regional Director in Los Angeles, met with interested individuals and shared how ACCA can support them and their loved ones. More than 100 seniors attended the Sankofa Elders Project resource fair which was held in conjunction with their November convening of ambassadors who have joined together monthly since April to share experiences and ideas with the goal of creating a Community Manifesto for the Care and Belonging of Black Elders. The Sankofa Elders Project seeks to identify and advocate for equitable resources and community solutions to address health disparities and increase the care, health, and wellbeing of older Black adults. ACCA was blessed to be a part of their event.

Team Member Tuesday: Meet one of our Care Navigators, Edgar Ibanez! Edgar is a member of our care team serving both English and Spanish-speaking participants in the greater Bay Area. Edgar joined the team this summer and quickly made a big impact on the team and participants alike. He quickly jumped in meeting participants at resource fairs and reaching out to new referrals. His willing spirit shows up in every aspect of his work and we are so blessed to have Edgar on the ACCA team!

On Sunday, November 10, ACCA held a special Caregiver Celebration at St. John Missionary Baptist Church in East Palo Alto. Videos explaining how ACCA supports caregivers through our Workbook for Caregivers were shown and gift bags were shared with caregivers as a token of appreciation for the work they complete every day for their care recipients. Many individuals signed up for follow up and additional information from our Care Navigators. November is National Family Caregivers Month and we continue to celebrate, uplift, and support Caregivers especially during this month!

Team Member Tuesday: Meet one of our Care Navigators, Nikki High! Nikki is a member of our care team serving persons needing care and caregivers in Los Angeles County. Our Navigators are the heart and soul of our program, working alongside participants to identify needs and learn skills to help them reach personal goals and improve their lives. We are grateful that Nikki is a member of the ACCA Team!
