🧩 National Case Management Week | October 11–17

Healthcare can be complicated. Appointments, medications, specialists, insurance, transportation, follow-up care—and sometimes housing, food access or other challenges outside the healthcare setting.

That’s where case managers can make an enormous difference.
During National Case Management Week, we recognize the nurses, social workers and other professionals who help patients and families navigate care, overcome barriers and connect with the resources they need.

Their work often goes far beyond making a referral. Research examining more than 7,000 patients found that many health and social goals required case managers to work with patients over months, highlighting just how much persistence and follow-through effective case management can require.
