A new paper using the SOSAS (Surgeons OverSeas Assessment of Surgical need) was recently published in JAMA Network Open. An improvised explosive device -- the most common weapon used against US forces in Iraq and Afghanistan -- detonated near a squad patrolling on foot in the a small dusty village. One soldier was severely wounded. The explosive blast tore apart his legs; both were traumatically amputated. An urgent cesarean section saves the child and prevents her from bleeding to death. A few months later the child repeatedly projectile vomits after eating. Pyloric stenosis, a muscular blockage of the small intestine, is diagnosed. An operation eliminates the blockage. [ 237 more words ]

Without the relatively simple operation the father of three and main breadwinner would certainly have died. He presented to the hospital like so many other patients in low and middle income countries (LMICs) with nearly dead intestines stuck in a defect in his abdominal wall – a condition known as a strangulated hernia. In high income countries such conditions are uncommon and rarely fatal; usually hernias are repaired early to prevent strangulation. [ 283 more words ]
