Prisoner to Patient We are a diverse group focused on bringing patients and professionals together to find the best ways We need your input!

We are a diverse team of patients, caregivers, and professionals from a wide variety of backgrounds. Our team is made up of formerly incarcerated persons, health professionals, community organization representatives, attorneys, reentry specialists, and violence prevention advocates. We are working together to identify how to best serve the health care needs of formerly incarcerated persons as they

come home and transition back into the community. We are committed to involving patients, caregivers, and a wide variety of stakeholders in developing our work!
