Are you a community health worker (CHW), CHW supervisor or CHW employer in Louisiana? If so, we want to hear from you!

Not sure if you are a CHW or work with CHWs? CHW serve as a bridge between communities and health and social systems. They have many titles including community health representative, community health navigator, promotor(a), community care coordinator, patient advocate, outreach specialist, health liaison, peer educator, member representative, and community health educator.

It’s important for policy makers and health professionals to understand the important work that CHWs do! That’s why LSU Health New Orleans and the Louisiana Community Health Outreach Network (LACHON) are working together to learn more about CHWs in Louisiana.

We invite you to participate in a brief 10-15 minute survey to help us learn more about CHWs in Louisiana. People who complete the survey will be entered into a drawing for one of five $50 prepaid debit cards.
