😿Coal is a 16 year old male who was surrendered by his owner on 6/4 due to concerns about his declining health. Prior to surrender, his owner reported excessive thirst, increased urination, and significant weight loss. Upon examination in our care, Coal was diagnosed with diabetes mellitus and is suspected to be experiencing diabetic ketoacidosis (DKA).
Coal urgently needs rescue placement by 6PM on 6/6 so that he can receive hospitalization, a comprehensive medical workup, and the specialized care he needs.
Medical
6/5 Doctor Exam
Surrendered due to decline over the past few weeks. Owner reports PU/PD and weight loss. Attempted to take to the vet twice but was unable to be seen. Quiet, alert, responsive. Moderately dehydrated. Mild mucoid ocular discharge OU. No nasal/aural discharge. Normal cervical palpation. Moderate dental calculus, no apparent oral masses or foreign bodies. Muscle wasting along epaxials. Ambulatory x4, no lameness. Brief orthopedic exam performed. Unthrifty fur coat. Doughy abdomen on palpation, firm stool in colon.
Body Condition Score (BCS): 5/9
Muscle Condition Score (MCS): 2/3
Assessment
- Diabetic, suspected DKA
- Moderate dehydration
CBC/Chem17/Lytes: lymph 0.28 (L), Glu >686 (H), BUN 72 (H), Tbili 1.0 (H), Chol 331 (H), T4: 0.8
6/4 Exam
BAR. Noted crusted discharge from eyes, particularly OD. Ears waxy. Moderate dental tartar and staining. Hair coat somewhat dirty/unkempt. Poor body condition, moderate muscle wasting, slightly distended abdomen, soft when palpated. Ravenous when offered wet food. No m/a on auscultation of heart. Primary DDx renal dz vs diabetes vs other, P is comfortable and stable, plan to set up in supportive care overnight, vet check in AM.
