LifeSpan Resources' Nutrition and Wellness Department has been busy this month! First photo - Tammy Snow, LSR's new Nutrition and Wellness Coordinator and two community members, completed Dementia Live Training. The remaining photos were taken earlier this month at the YMCA of Harrison County where they are hosting a Stepping On Falls Prevention Class. LifeSpan Resources has partnered with the Y, Harrison County Health Department, Anna Jones, RN, Iris Wiseman, PT, Rehab Services Manager at Harrison County Hospital and Sophia Jacobi, Director of Sales at Harrison Springs Health Campus to host this 7 week Falls Prevention class for the community.

What is Chronic Care Management (CCM)?

If you have two or more chronic health conditions—such as diabetes, arthritis, heart disease, or high blood pressure—that are expected to last at least 12 months or for the rest of your life, you may qualify for Chronic Care Management (CCM) services through Medicare.

CCM is a legitimate Medicare benefit that helps your health care provider coordinate your care between office visits. Services may include:
✔ Creating and updating your personalized care plan
✔ Managing your medications
✔ Coordinating care with specialists and other providers
✔ Monitoring your ongoing health needs
✔ Providing support to help you manage your chronic conditions

Before CCM services begin, your provider should explain the program and obtain your consent. You may also be responsible for a copayment unless you have supplemental coverage.

Always review your Medicare Summary Notice (MSN) or Explanation of Benefits (EOB) to make sure you recognize any CCM charges. If you see services you didn't receive or don't remember agreeing to, contact your local Senior Medicare Patrol (SMP) for free, confidential assistance.

Knowing what Medicare covers is one of the best ways to protect yourself from fraud, errors, and abuse.
