“Instability has been solved with the anterior approach.”

Well… maybe not quite.

In the second AHF Surgeon Roundtable, Drs. Jeffrey Barry, MD, blair ashley and Chancellor Gray join host Joseph Schwab to discuss what actually drives instability after anterior THA, and what surgeons can do about it.

The conversation gets into cup position, offset, soft tissue handling, hip-spine mechanics, dual mobility, and those intraoperative moments when something just doesn’t feel right.

There are also some very different perspectives around the table, including how aggressively to plan around patient risk, what stability checks really matter, and when to escalate the construct.

One line from Chancellor Gray sums up the mindset well:

“I want to leave the OR with a hip that I trust, even if the patient does something pretty stupid.”

Because ultimately, the goal isn’t a hip that only works when everything goes perfectly. It’s one that can tolerate real life.

Revision hips have a habit of turning a good plan into a very different operation.

In this Revision Track presentation, Raul Gosthe, M.D. talks through the obstacles that can derail a case, and the practical techniques he uses to keep moving:

• Broken screws blocking reaming

• Constrained and dual-mobility liners that won’t come out

• Cups that can’t be reached with a standard explant

• Complex intrapelvic migration requiring a very different Plan B

His message is simple:

“If you’re thinking too much in surgery, it’s because you haven’t prepared enough.”

Preparation means knowing the implants, anticipating the instruments you may need, getting the exposure right, and recognizing when it’s time to stop fighting Plan A.

Most surgeons can plan a hip replacement down to the millimeter ... and confirm the result afterward on X-ray.

It's the part in between where things get fuzzy.

That gap is what pushed Dr. J Dean Cole and Scott Banks to build Orthopedic Driven Imaging, the company that just won Shark Tank at hashtag in Nashville.

Their system runs on the GE C-arms already in most ORs - with a wider field of view, less radiation, and no new workflow to learn.

In the latest AHF Podcast, they tell host Joseph Schwab why "your X-ray looks great" isn't good enough for patients who are still unhappy with their outcome, and why robotic platforms with beautiful planning software still leave surgeons aiming at the wrong target.

The foot came out of the boot. The leg hit the floor. And a difficult femur became a bad day in the OR.

In the latest Operation FUBAR edition of the AHF Podcast, Dr. Nicholas H Mast joins host Joseph Schwab to walk through eight challenging hips- including the case that “definitely took years off” his life.

More screws won’t fix a missing anterior column.

In severe “up and in” acetabular defects, more than half the revision cup may be unsupported, and even long posterior column screws may not stop it from migrating back through the defect.

Our own Jonathan Yerasimides, MD, shows how he reconstructs rather than spans the defect, using intrapelvic augmentation to create a strong, unified construct.

That question came from the surgeon who trained the man standing opposite him.

At AHF 2026, Aldo Riesgo and John Masonis went at one of the biggest questions in revision arthroplasty: should we expand the indications for anterior revision THA, or respect the limits?

Aldo went first. He credited John with teaching him to push boundaries — then spent four minutes arguing against him.

John didn't argue that it can't be done.
He asked whether it was always optimal.

Hip replacement has been performed for more than 70 years. So why don't we actually know how it's done around the world?

Approach. Fixation. Bearing surfaces. Positioning. Pre-op and post-op pathways. Practice varies wildly between countries, institutions, and individual surgeons - and in much of the world, registry data is incomplete, inconsistent, or missing entirely.

In this episode of the AHF Podcast, indefatigable host Joseph Schwab sits down with Irrum Afzal (Imperial College London) to talk about the Global Arthroplasty Practice Survey (GAPS) and the effort, with Professor Richard Field and an international group of surgeons, to map how hip replacement is really being performed worldwide.

Not how we assume it's done. Not how one registry captures it. How surgeons actually practice.
