Collabria Care has completed our integration with Providence and we are excited to share our new website and brand with you. (Soon Facebook will approve our name change too!)

Since we were founded as Hospice of Napa Valley in 1979, our South Jefferson Health Center has become a leading provider for those facing the transitions of aging, Alzheimer’s disease and other serious illnesses or the end of life, throughout Napa County. Our services are now offered under:
