The legal exposure in running an outpatient lab is not the part anyone trains for, and it is the part that ends practices.

Part one of our 2026 legal session is on the OEIS YouTube channel. It is close to an hour and it is worth the time if you are operating a lab or about to.

What it covers:

- Where the regulatory landscape actually moved this year
- Structural decisions that create exposure later
- The compliance questions that come up in every real-world lab

This is the least glamorous hour on our channel and probably the most useful one, particularly with federal attention on the setting increasing.

What does it take to speak for an entire specialty?

At the 2026 OEIS Annual Meeting, a multidisciplinary panel brought together representatives from SCAI, SIR, and SVS to discuss what it really means to represent a medical society in the public eye.

Dr. Arnold Seto, Chair of the SCAI Advocacy Committee, put it plainly: one quote, one interview, one moment, and it could land on the front page of the New York Times.

That kind of responsibility demands more than clinical expertise. It requires:

✅ The ability to set aside personal opinion in favor of collective voice
✅ A global perspective on how statements will be received across the entire medical community
✅ The awareness that we are pioneers, representing a specialty that is still defining itself

Outpatient endovascular care is still a relatively small community. That makes every word matter even more.

OEIS is building the advocacy infrastructure that outpatient IR and vascular surgery deserves, and we are looking for leaders who are ready to be part of that work.

A question for the physicians in this community who have already built one.

If you opened your lab again tomorrow, knowing everything you know now, what would you do differently?

We hear the same few answers more often than the rest. Hired the wrong first person. Underestimated how long credentialing would take. Signed a payer contract without modeling the rate. Bought equipment before the volume justified it.

But the useful answers are usually the specific ones nobody warns you about.

If you are eighteen months out from opening, the comments on this post will be worth more than most of what you will read this month.

Uterine fibroid embolization has decades of evidence behind it. Most women with symptomatic fibroids still never hear it offered.

That is not a clinical failure. It is a referral one.

The pattern is consistent. A patient presents to gynecology, the conversation moves toward hysterectomy or myomectomy, and a uterine-sparing option delivered by an interventional radiologist never enters the room. Not because anyone opposes it, but because the pathway was never built.

What closes the gap tends to be unglamorous:

- A named IR contact a gynecology practice can actually reach
- Notes that go back on every single patient
- Being honest about which patients are not good candidates
- Shared clinic time rather than a one-way referral request

Fibroid care is one of the clearest cases where the outpatient setting serves patients well and the referral infrastructure has not caught up to the evidence.

On the BackTable Cardiology podcast, Dr. Fadi Saab was asked where physicians who are not well-versed in business should start. His answer was not a reading list. It was a room. 🎙️

A meeting like OEIS' Annual Meeting gives real program time to the parts of practice nobody covers in fellowship: regulatory process, legal process, auditing, and what it actually takes to stand up a new service line.

Two things happen when you show up:
