Shout it out to the world Sarah Berry! Childbirth and menopause, two of the most significant life events women experience, are the leading causes of pelvic organ prolapse. Heavy lifting, hard foot strike fitness activities, weak tissue integrity conditions such as Ehlers Danlos Syndrome, chronic constipation, genetics, neuromuscular disease, the list goes on and on. When we consider all of research validated lifestyle and behavioral causes that ripple through every woman's world, it's a wonder POP prevalence is not at 100% of us.

What say you urogynes?

Gold standard sacral colpopexy vs the new kid on the block lateral suspension to address va**nal vault prolapse (the top of the va**na caving in).

Sacral colpopexy is a surgical procedure used to treat pelvic organ prolapse by lifting the top of the va**na back into its normal position and attaching it to the sacrum (tailbone) with a supportive mesh.

Lateral suspension is a keyhole surgery used to lift a caved in va**na back into its normal position by attaching it to the side muscles of the lower belly using a special mesh.

*Both methods support the va**nal canal but from different directions.
*Both methods utilize mesh.
*Success results at the top of va**na and front va**nal wall close to equal.
*Sacro takes longer to perform.
*Presacral dissection is needed for sacro method, nerve damage risk is avoided with lateral.
*Mesh complications low with both procedures; a tad lower with lateral.
*Long-term efficacy with lateral is not yet well established.
*Less value for rectocele repairs.

So...sacral colpopexy is the better-established, long-term benchmark vs
a promising less-invasive alternative. Is lateral the wave of the future, or will more studies prove it to have less long-term efficacy?

It's so important women ask their surgeons how many laterals they've performed, what has their success rate been to date, and would they provide this surgery on their mothers over sacral colpopexy?

Women with pelvic organ prolapse who have an exercise regimen they are used to and love are often disillusioned having to change their normal fitness routine. Hard foot strike fitness activities can jerk organs downward if the pelvic floor structure is weak due to childbirth damage or loss of estrogen with aging or menopause. It's a bit of double-whammy as well for those that also have osteoporosis bone loss issues. And as ever, I worry about our following that have Ehlers Danlos Syndrome weak tissue integrity.

What to do?

It's important to recognize every body is unique. Age, lifestyle, behaviors, fitness level, health conditions impacting general health all must be considered. Always seek counsel with your healthcare provider. But here is a relatively generic and safe list to consider. For those with access to a pool, swimming should be included in this list but I couldn't fit it on the screen. Organs float easing pressure on the pelvic floor, and all long muscles get exercised!

Yes women have testosterone. And oh-by-the-way, men have estrogen.

When I was a teenager, I tested out at an above normal level for testosterone. Really notable acne pizza face. No periods. I wasn't crazy about testosterone back then. But now I love it. I love how it makes my muscles look ripped from working out. I love that it enhances my strength a tad. And libido enhancement is a perk for sure. I'm taking my testo cream along for my final ride someday since no one really knows how to pack for that trip and maybe I can find a fitness center on the other side of the great divide. :)

Testosterone for women can have benefits far beyond sexual desire. New research is theory until validated, and long-term safety is important to establish. However it is very exciting to see tire-kicking explorations expand in testosterone use in women to potentially enhance:

*genitourinary health,

*va**nal lubrication,

*tissue integrity,

*and to a smaller degree, cardiovascular health.

Exploration is moving forward from does testosterone increase libido specific toward what does testosterone cellular signaling actually do within the female body. We need to understand so much more about the menopause androgen - connective tissue strength and integrity depletion backdrop.

Co**al incontinence (urinary incontinence during sexual activity) is not a niche or uncommon type of incontinence. CI occurs in an estimated 35% of women experiencing urinary incontinence yet it remains underreported and inconsistently screened for.

Co**al incontinence can occur at different points:

1. At pe*******on.

2. At or**sm.

3. At both pe*******on and or**sm.

The question I have is how many women experience co**al incontinence and don't even realize they are experiencing urinary leakage? Maybe think they are just really wet due to stimulation lubricating the va**na?

Despite the silence encasing pelvic organ prolapse, women have experienced and reported POP concerns to medical providers thousands of years prior to modern urogynecology.

The recommended treatment for uterine prolapse involved a mixture of oil of earth (naturally occurring unprocessed crude oil), animal dung/manure, and honey that was rubbed onto a woman's body.

Certainly not an effective treatment for pelvic organ prolapse, but I'm guessing it likely created a roadblock to experiencing pain with intimacy. 😮

What are peptides?

Peptides are strings of molecules called amino acids, which are the "building blocks" of proteins. Basically short proteins that are about 2-100 amino acids long. Peptides may have anti-aging, muscle growth, and fat loss benefits.

While media currently is mostly waxing on about the fat loss side of the equation, our interest in the pelvic organ prolapse space is the strengthening and restructuring for repair of soft tissues.

Rather than viewing this from the put something there to mechanically hold things up.

Should we be asking:

Can we create an environment in which the patient's own cells rebuild better tissue?

There is so much fear in the pelvic organ prolapse space. When women experience the pretty radical symptoms of POP, symptoms they've never heard of before, they assume it is only them. Organs are attached on the inside, they will not fall out and land on the ground. They push out the va**nal opening but remain attached on the inside.

We must increase POP awareness by any and all means possible. There are so many more practitioners talking about POP on social media than there were even 3 years ago, much less 16 years ago, but we also need women sharing their experiences out loud.

Pelvic Organ Prolapse is the Biggest Secret in Women's Health. Let's join hands to fix that.

Pessary fitting is an art, zero doubt. Is a failed pessary fitting a treatment failure or failure to match the appropriate pessary to a patients pr*****ed anatomy?

There are many reasons pessaries are difficult to fit; our bodies are all unique. There can be one issue or a few issues.

Advanced POP, particularly Stage 4. Insufficient va**nal anatomy for a ring pessary to stay in place, (Stage 4 does not mean pessary treatment is always impossible).

Wide va**nal introitus. A pessary may not have enough tissue support to remain in place.

Large ge***al hiatus. A larger opening can make retention more difficult.

Short total va**nal length (hysterectomy can shorten. Insufficient va**nal length for the pessary to stay in position securely.

Posterior-compartment prolapse. Rectocele can make a pessary less effective at relieving symptoms.

Levator ani avulsion. Avulsion is associated with a substantially increased risk of pessary pushing out.

Previous prolapse surgery. Altered va**nal anatomy can make fitting more difficult.

Previous hysterectomy prior to POP repair can result in more difficult fitting success.

Higher BMI. Yes, excess weight matters.
