What does a Supports Broker actually do?

At OnBoard Outreach, we help individuals and families navigate the many moving pieces that come with finding and coordinating the right supports.

We can help with things like:

✔️ Understanding service options
✔️ Connecting with providers and community resources
✔️ Paperwork, renewals & follow-ups
✔️ Identifying gaps in services
✔️ Problem-solving when things aren't working
✔️ Goal planning & building independence
✔️ Resource and life navigation

And right now, we have plenty of openings! 🙌

Many families don’t know where to begin when it comes to supportive housing, resources, and planning for future independence — and that’s okay.

Support Brokerage may be able to help guide individuals and families toward resources, supportive living options, and next steps throughout the process.
