lifEvolv is a Prenatal Care Coordination (PNCC) - Medicaid and BadgerCare Plus benefit that helps pregnant women get the support and services they need to have a healthy baby. lifEvolv services include:
• Help getting the health care you need
• Personal support
• Information on good eating habits and health practices
• Help finding needed services in your community
lifEvolv's primary purpose is to provide access to medical, social, educational, and other services to pregnant women who are considered high risk for adverse pregnancy outcomes. The components of this benefit are outreach, assessment, care plan development, ongoing care coordination and monitoring, and health education and nutrition counseling. Some key outcome indicators include: To***co Exposure, Alcohol Use, Breastfeeding, Safe Infant Sleep Practices, Perinatal Depression, Family Planning, and Involved Father.
Duties will include but not limited to:
• Monitoring of high-risk pregnant women
• Performing health education counseling with high-risk pregnant women, to include smoking, drugs, alcohol, and nutritional counseling.
• Assessing psycho-social risk factors for pregnant women
• Assessment and scoring of high-risk factors for program eligibility
• Supervising client risk assessment and ongoing care coordination and
