Home Care AFC provides Adult Family Care (AFC) services in Massachusetts and Connecticut for individuals aged 16 and older with disabilities or medical conditions that prevent them from living independently.

The AFC program supports families who are already caring for a loved one at home—or who may be considering this option. It does not recruit outside caregivers; rather, it strengthens an existing caregiving arrangement by providing structured clinical oversight and financial support.

Through the AFC program, families may receive:
Up to $18,000 per year (tax-free) for the caregiver
Monthly in-home visits from a nurse and case manager for ongoing support. Ongoing guidance with care coordination, referrals, navigating health benefits, and connection to community resources.
