East Bay Veteran Community Partnership- EBVCP The EBVCP (formerly EBHVP) is a non-profit volunteer organization under the We Honor Veterans national program.

The Issue

The Veteran's Administration has determined that only 4% of Veterans are being served at end of life in VA facilities. The remaining 96% are being cared for at end of life in community hospices, nursing homes, and other venues. Goals and Objectives

* Identify and resolve system issues to improve Veteran/family access to timely hospice care in the setting of their choice.

* Ensure that

VA facilities and community hospices develop on-going relationships to provide quality EOL care for Veterans.

*Conduct outreach in our communities and to VSO's, providing education regarding hospice and palliative care services.
