We’re excited to be at the Ashland County Community Health & Wellness Day today!

If you’re in the Ashland area, stop by UH on Main between 2:00 PM and 6:00 PM to connect with local organizations, learn about community resources, and meet the people working every day to improve the health and well-being of Ashland County.

Our team will be there sharing information about CHAP’s Community Health Worker services and how we help connect individuals and families to the care and resources they need.

A big thank you to everyone who helped organize today’s event and to all of the community partners and vendors who are making it possible. We look forward to seeing you this afternoon!

We're grateful for organizations like Shelby Help Line Ministries that are opening their doors to the community while air quality remains poor due to wildfire smoke.

This is what strong community partnerships look like.

We're fortunate to work alongside organizations like the Erie County Health Department that don't just respond to public health issues, they get ahead of them. As wildfire smoke continues to impact air quality across Ohio, they're making N95 masks available at no cost to help protect their community.

Public health isn't always flashy. Sometimes it's as simple as making sure a family has the resources they need before a problem becomes a crisis.

Thank you to the Erie County Health Department/Erie County Community Health Center for continuing to lead with prevention, accessibility, and a commitment to the people you serve. We're proud to call you a Community Care Agency partner.

Notice the hazy skies today? 🌤️ Wildfire smoke drift is impacting our air quality here in Erie County. Standard masks don't block these fine smoke particles, but we’ve got you covered.

Starting today, we are giving away FREE N95 masks to all residents at our main health department campus! 😷👇

📍 Pick-up Location: 420 Superior Street, Sandusky
🛑 Cost: 100% Free (No ID or appointment required)

We're growing!

The Community Health Access Project (CHAP) is looking for our next Director of Hub Operations.

This is a leadership role for someone who enjoys solving problems, building systems, supporting partner agencies, and helping drive our mission forward. The person in this position will play a key role in training, quality improvement, operations, and ensuring our Community Health Workers have the support they need to make a difference across the communities we serve.

If you're passionate about improving community health, leading teams, and creating meaningful impact, I'd encourage you to take a look.

This is not a sit quietly behind a desk role. We're looking for someone who is collaborative, adaptable, and ready to help shape the future of CHAP.
