CHAP Office Closure Reminder

Just a friendly reminder that the CHAP office will be closed on Friday, June 19 in observance of Juneteenth.

Our team will be back in the office and available during normal business hours on Monday, June 22.

As always, if you're a current participant and have an urgent need, please reach out directly to your Community Health Worker.

We're growing!

The Community Health Access Project (CHAP) is looking for our next Director of Hub Operations.

This is a leadership role for someone who enjoys solving problems, building systems, supporting partner agencies, and helping drive our mission forward. The person in this position will play a key role in training, quality improvement, operations, and ensuring our Community Health Workers have the support they need to make a difference across the communities we serve.

If you're passionate about improving community health, leading teams, and creating meaningful impact, I'd encourage you to take a look.

This is not a sit quietly behind a desk role. We're looking for someone who is collaborative, adaptable, and ready to help shape the future of CHAP.

Today, CHAP had the privilege of recognizing and celebrating the Community Health Workers who serve our communities every day.

Community Health Workers are often the bridge between healthcare and the people who need it most. They build trust, navigate barriers, connect individuals to resources, and improve outcomes in ways that are both measurable and deeply personal.

Thank you to Richland Source for covering our Community Health Worker Day celebration and helping shine a light on this incredible workforce. I am grateful to our partner agencies, community leaders, and elected officials who joined us in recognizing the impact Community Health Workers have across North Central Ohio.

Most importantly, thank you to the CHWs. Your work changes lives, strengthens communities, and makes our healthcare system better.

A phone call, a ride to an appointment or help finding the right resource can make all the difference for someone facing a difficult situation.

Today, we celebrate the people who make this work possible.

Community Health Workers are often the first person someone trusts, the first person who answers the phone, and sometimes the first person who truly listens. They meet people where they are, walk alongside them through some of life’s toughest moments, and help turn barriers into opportunities.

The impact of a Community Health Worker can’t always be measured in numbers. It’s found in healthier babies, stronger families, stable housing, access to care, and lives changed because someone took the time to show up and help.

At CHAP, we believe Community Health Workers are one of the most powerful forces for improving health and strengthening communities. Every day, they prove that real change happens through relationships, trust, and a commitment to helping others succeed.

To every CHW across Ohio and beyond: thank you for the work you do, the lives you touch, and the difference you make. Your work matters. Your voice matters. Your impact matters.

A special thank you to Buckeye Imagination Museum for graciously allowing us to take over their space for today’s celebration, and to our partners at UnitedHealthcare, AmeriHealth, and the many partner agencies that donated gifts and helped make this day possible. The Community Health Access Project (CHAP) is looking for an Operations & Training Manager to help support and strengthen our Community Health Worker network across North Central Ohio.

This role is ideal for someone who enjoys building systems, supporting people, solving problems, and helping teams grow. The position works closely with CHWs, supervisors, and community partners to support training, operations, quality improvement, and day-to-day program success.

At CHAP, our work is centered around people. We believe healthcare starts long before someone walks into a clinic, and we’re looking for someone who shares that mindset.

CHAP is at the 27th Annual Minority Health Fair from 10:30 AM – 2:00 PM at Providence Baptist Church on 6th Street in Mansfield.
