We’re hiring!

The Community Health Access Project (CHAP) is looking for an Operations & Training Manager to help support and strengthen our Community Health Worker network across North Central Ohio.

This role is ideal for someone who enjoys building systems, supporting people, solving problems, and helping teams grow. The position works closely with CHWs, supervisors, and community partners to support training, operations, quality improvement, and day-to-day program success.

At CHAP, our work is centered around people. We believe healthcare starts long before someone walks into a clinic, and we’re looking for someone who shares that mindset.

We’re really excited to share a new partnership with NAMI - Richland County here in our community.

Mental health is often one of the biggest barriers people face, and one of the hardest to navigate alone. That’s why this partnership matters.

NAMI has long been a trusted voice, offering support, education, and connection for individuals and families walking through mental health challenges.

By coming together, we’re able to strengthen that support by connecting individuals not just to resources, but to real people who will walk alongside them. Through our Community Health Workers and NAMI’s peer-led support, we’re building a more complete path to care.

This is what it looks like when a community shows up for each other.
This is neighbors connecting neighbors.

We’re excited to share some news with our community.

The Community Health Access Project (CHAP), in partnership with UnitedHealthcare, has launched a new initiative to provide free, discreet pregnancy test kits at trusted locations across North Central Ohio. The goal is simple: help individuals identify pregnancies earlier and connect to care and support sooner.

Each kit includes a pregnancy test and information on how to connect with one of CHAP’s Community Health Workers for confidential support. Once someone knows they’re pregnant, our team can help connect them to prenatal care, transportation, nutrition resources, insurance enrollment, and other services that support healthy moms and healthy babies.

Early care makes a difference. When families are connected to support sooner, it can improve outcomes for both mother and baby.

If your organization serves the community and would like to make pregnancy test kits available at your location, we would love to partner with you. Please reach out to CHAP at 419-526-2427.

We’re excited to welcome NAMI - Richland County as the newest Care Coordination Agency (CCA) in the CHAP HUB network!

NAMI has long been a trusted voice for mental health advocacy, education, and support in our community. By joining the HUB, their team will now be able to connect individuals and families they serve with Community Health Workers who help address barriers related to health, housing, transportation, food access, and more.

This partnership strengthens our ability to support the whole person. Mental health and physical health are deeply connected, and working together allows us to better serve individuals who may be navigating both.

We’re grateful to have NAMI join this work and look forward to the impact we’ll make together.

We’re excited to welcome our newest Care Coordination Agency (CCA) partner, Ashland Church Community Emergency Shelter Services - Access to the CHAP Pathways HUB network!

Ashland Access is a nonprofit organization working to break the cycle of homelessness and poverty by helping stabilize women and families in Ashland County through safe housing, life-skills development, and supportive services.

By joining the CHAP network, Ashland Access will work alongside our Community Health Workers and partner agencies to identify individuals facing barriers and connect them to the resources and care they need to thrive.

Partnerships like this strengthen our shared mission to ensure families across North Central Ohio have the support they need for healthier, more stable futures.

CHAP is currently looking for a Billing Specialist to join our team.

At the Community Health Access Project (CHAP), we work alongside community health workers and partner agencies across North Central Ohio to help individuals and families overcome barriers to care. Our work focuses on connecting people to resources and making sure those connections lead to real outcomes.

The Billing Specialist plays an important role in supporting that work. This position helps manage invoicing, track payments, and support the outcome-based billing process that allows our partner agencies to be paid for the work they do in the community.

If you enjoy working with numbers, systems, and being part of a mission-driven organization, we’d love to hear from you.
