Daniel G Sleva, Jr Memorial Foundation, Inc. We are a 501(c)3 non-profit organization . Daniel G. Sleva, Jr. It is his love of life and giving that this Foundation was created in his name. The Daniel G.

Our mission is to help those with Type 1 Diabetes obtain the necessary medical supplies needed to live as normal a life as possible by he was a very caring, loving and giving young man who loved and cherished his catholic faith, his family and his friends. Memorial Foundation has been created to offer awards to various High School Seniors who demonstrate the same qualities Daniel did as a person.

There are 3 awards to high school seniors that will be offered on an annual basis. A Gift of Giving will also be done to help supply diabetic supplies to those in need. The awards and gift giving are as follows:

#1 - Alma Mater Award: To a High School Senior in Daniel's Alma mater, Mahanoy Area High School. This Senior will be involved in various aspects of school functions and be in (a) Special Education class(es). He or she will be chosen via recommendations of the Special Education teaching staff.

#2 - Leadership Award: To a High School Senior who is involved in the A.C.T.I.O.N Youth Group and demonstrates the same qualities Daniel did as a member of this group. This Senior member will participate in all aspects of the youth group and will be recommended via the Director of A.C.T.I.O.N.

#3 - Faith Award: To a High School Senior within St. Richards Church, Barnesville, PA. This Senior will be a dedicated Alar Server in St. Richards who demonstrates the same qualities Daniel did when he served his faith. He or she will be chosen via recommendation of the current Priest.

#4 - Gift of Giving: This gift will be used to purchase needed diabetic supplies for others who fight Type 1 Diabetes, just as Daniel did. We will work with Doctors, Pharmacies and Medical Supply companies to help get those in need get the medical supplies needed to help them live as healthy of life as possible. (This gift will be given on an as needed basis and as long as it is possible via fundraising efforts.) Please include a Self-Addressed Stamped Envelope for a return receipt. Our mission is to help those with Type 1 Diabetes obtain hte necessary medical supplies needed to live as normal as life as possible. We will help with co-pays and deductibles for those that are working and have health insurance.
