PATCH — Providers and Teens Communicating for Health

We’re a community of teens and adults working together to support adolescent health and wellbeing. Our sweet spot is making sure teens have a voice:
- In health care settings
- In decisions that shape teen health
- In their own lives

We also help adults and organizations meaningfully engage teens. Whether replicating our work or learning from how we partner with teens, we love sharing what we’ve learned and offering practical guidance.

Earlier this year, PATCH HQ took on some (slightly new to us) work which allows us to more directly focus on CYSHCN (children and youth with special health care needs) and mental health.

Things we’ve always cared about but were semi on the outside of the statewide work.

Being more involved has introduced us to a whole new group of partners doing sah-wheet work that overlaps with ours. And we’re pretty excited about it.

Our Mental Health & Inclusion Specialist (a newly created position!) has stepped in to lead quite a few projects.

Jay’s running a mini-grants project with our WI PATCHies, bringing more intentional mental health and CYSCHN content into our programming, joining a variety of statewide workgroups, and overall just making sure our work continues to serve all teens.

Jay also was nominated to become a Wisconsin Children’s System of Care Champion! So, he’s been guiding counties through a self-assessment tool to celebrate their strengths and also find opportunities to improve so that children and families get timely and coordinated support among community orgs (like, education, child welfare, youth justice orgs, and those supporting physical and mental health).

At their recent retreat, Champions were awarded beautifully crafted by indigenous artists at Bam'idizowigamig Creator's Place! Each uniquely designed based on their individual bios. Look at Jay’s! 🥹 So. Freakin’. Cool.

First enrichment meeting for our newest PATCH Madison team is in the books! 💙

Felt great to settle back into the space, eat some grub (thanks Glass Nickel Pizza Co. - Madison East for the generous donation!), and get back into the groove.

Jay from PATCH HQ joined us to introduce a new project we’ve got going for this program year. 😏.

And, naturally, we had to throw in a few getting-to-know-each-other, figuring-out-how-we-work-together activities.

Like, who can build the tallest tower with noodles and marshmallows in 16 min flat?! 28 inches was preeetttyyy impressive!

Love that there’s built-in space and time to be together as a team. To learn, chat, and dig into the stuff that helps us do the work well.

Lots to get into this year: MyChart. Healthy relationships. Public policy + advocacy. ...it’s all coming!

BTS: WI Teen Consultant Training // Day 2

Eat. Work. Eat. Work. Eat. Work. Free time! [insert teen-initiated, impromptu talent show, sing-a-long, + dance ppaaarrrtttyyy! 😏]

It's a loooong day (8:30a–8p!) of training, so we're sure to switch things up. Often!

We had visits from our partners at WI DHS, PATCH Madison PATCHies(!), and most of our PATCH HQ staff.

Our day in a nutshell: (Re)discovering what we’re passionate about, spotting opportunities to improve the health and well-being of teens, and figuring out how to use our voices and stories effectively!

And, that evening free time! Golly. It made our hearts sssiiinnnggg. Figuratively. And, literally. 💕🎶
