We're getting excited about the Summit, but we're always excited about stewardship. As part of our mission to improve patient outcomes and advance antimicrobial stewardship in every part of Arkansas, we're not only having a virtual option for the Summit this year, but we are piloting a virtual series as we ascend to the summit. We've decided to start the series on USAAW.

More information to come, but be sure to save the dates. I'll send everyone on our mailing list information about registration as soon as we have it!

Our residency cards are done, but I'm not done sharing information. IDSA updated their Gram negative guidance in July. The most significant updates were adding some new drugs that were FDA approved since the last update and a big change in the Steno and CRAB recommendations. I know we just got new Surviving Sepsis guidelines, but did you see the new drop from IDSA/ACEP/ASM and several other orgs published in Clinical Infectious Diseases Wednesday? They contend that our traditional "bundle-based" approaches have shown inconsistent results in clinical outcomes. Key takeaways include:

Six domains of care: The guidance spans diagnostics, antimicrobial management, surveillance, adjunctive therapy, infrastructure, and infection prevention.

Practical implementation: From optimizing blood culture workflows to standardizing antibiotic infusion times and implementing "sepsis governance" structures, this paper bridges the gap between bedside care and hospital-wide quality frameworks.

Welcome to the last of our residency survival cards for 2026! I hope you've all survived your first few months of residency and are already on the way to becoming great stewards! Today, we're talking about synergy.

I may have an unofficial one next week because I'm working on a graphic with the new IDSA Gram - guideline updates, so do stay tuned for that one too, but synergy dosing does tie in a little (these may be a little ABOVE a residency survival card - you need to get an ID consult for those infections but I have seen pharmacy residents try to stop a "duplicate" antibiotic when it's really for synergy so synergy definitely is something we all need to be aware of).

You new grads probably learned all about the shorter durations, but just a short decade or so ago, we were treating everything for 2 weeks or more. Shorter durations are safer and just as effective. You might have to convince your older colleagues with some of the data!
