Delighted to have shared the stage at Mayo Rochester’s Transforming Community, Regional, and Rural Healthcare 2026 with champions for CHW work in WI and MN.

CHW Barriers to Breakthroughs, explored how CHW program models differ from state to state. Our biggest takeaway came from Evan, CEO of Wellshare: behavior change as success baked into their model. We’re looking forward to learning more from their work.

It's Community Health Worker Week!

We're taking a moment to brag on our team. Our CHW's are the ones who show up in homes, in the parking lot before the mobile clinic doors even open, finding sustainable housing for the unsheltered and at risk, and in and on...

They're the connective tissue between our patients and everything else - The stuff that doesn't fit neatly into an appointment slot but determines whether someone actually gets healthy.

A huge thank you to Nancy Page, who leads our CHW program with the kind of steady, patient leadership that makes everyone around her better at their jobs.

To every CHW on our team, thank you!

Today was Dr. John Porter’s last day with our Mobile Medicine Program. We are incredibly grateful for the years he has spent supporting our team and caring for patients in rural communities with our Mobile Clinic team.

Dr. Porter’s skill, compassion, kindness, and wisdom have made a lasting impact on our patients, our team, and on the program as a whole. We cannot thank you enough for everything you have done for our patients and our team, Dr. Porter! We wish you all the very best in your future endeavors - you are already missed!

We're looking for an AmeriCorps member to join us this fall as we transition to our new clinic facility and roll out a new electronic health record system. This full-time role focuses on operations, EHR support, and community outreach/volunteer recruitment, making it a great fit for a recent grad looking for a meaningful gap-year opportunity before graduate school, or for anyone looking to jump-start their career in public health and nonprofit work.
