Our tabling skills are on point! 🤩

Our outreach team has been out in the community this fall, connecting with neighbors and sharing information about Kinship Health!

Thank you to the organizations that invited us to be part of these events.

A care transition involves much more than paperwork.

It means listening to concerns, coordinating with care teams, finding resources, and helping patients and families understand what comes next.

As National Case Management Week wraps up, we want to recognize the people behind that work. To our case management partners: Thank you for your dedication and for keeping patients and families at the center of every next step. 💙

Navigating healthcare can feel overwhelming. Having someone who listens, explains the options, and helps coordinate next steps can make all the difference.

During National Case Management Week, Kinship Health thanks the hospital and skilled nursing case managers serving older adults and families throughout our communities.

We appreciate your advocacy, compassion, and commitment to the people you serve. 💙

PACE is about more than coordinating healthcare.

It is about helping older adults continue living in the homes and communities they love. It is about creating opportunities for connection, supporting caregivers and making sure participants have a voice in their care.

Every participant has a different story, different needs and different goals. That is why PACE care is built around the individual—not a one-size-fits-all approach.

As National PACE Month comes to a close, we celebrate everyone who makes this mission possible and the participants who inspire us every day.

Interested in learning whether PACE may be right for you or someone you love? Contact your local center to start the conversation.

September 21 - 25 is Falls Prevention Awareness Week.

Falls are not an inevitable part of aging, and a few simple changes at home can help make everyday life safer.

Keep frequently used items within easy reach. For objects that are too high or too low, use a reacher or ask for help rather than stretching or climbing.

This Falls Prevention Awareness Week, we’re sharing small reminders that can help older adults stay safe, confident and independent at home. 💙

What makes PACE different?

It starts with a team that truly gets to know each participant.

Doctors, nurses, therapists, social workers, dietitians, transportation professionals and other members of the care team work together to coordinate services around each participant’s individual needs and goals.

That means fewer disconnected pieces for older adults and their caregivers to manage, and one team communicating, planning and helping along the way.

During National PACE Month, we’re celebrating this team-based approach to care and the dedicated people who bring it to life every day.

September is National PACE Month! 🎉

Throughout the month, we’re celebrating the participants, caregivers, employees and community partners who make PACE possible.

PACE (Program of All-Inclusive Care for the Elderly) brings healthcare and supportive services together to help eligible older adults continue living safely and independently in their own homes. Through one coordinated program, participants can receive medical care, therapies, transportation, medications, personal support and opportunities for meaningful connection.

Most importantly, PACE helps older adults continue living life on their terms, surrounded by the people and places they know and love.

Follow along this month as we share some of the people, moments and stories that make our PACE community so special! 💙🧡
