Case management-
Case-management is the foundation of all services offered at New Beginnings Youth and Family Services. Every case is individualized to meet the specific needs of that client. Clients are assigned a case-manager who is the manager of their case. The case-managers job is to work one-on-one with the clients in helping them to meet their immediate needs, develop obtainable, strong t
ransition plans for their future, help guide them towards their goals and connect them with other non-profits and partners and collaborators in order to gain other resources needed in the community. Case-managers will meet with their clients monthly or as needed to help the clients stay on task and meet their goals. Case-managers will focus on immediate needs to include crisis intervention, housing, employment, health, and other needs that are immediate barriers for the client in reaching their full potential and independence. Case-management services will also be directed based on the State or Federal grant contract requirements. Case-managers have a bachelor’s degree in a social service field and go through intense training to gain the knowledge, skills and abilities to be an effective case-manager for their clients. Life skills-
Youth and families who are in need of life skills training will be able to attend a group or individual training to help them develop skills needed to meet their goals. Life skills are crucial to help clients develop healthier skills sets and challenge faulty thinking that are barriers to their success and goals. Life skills training include, parenting skills, personal/interpersonal skills, job skills, health and safety, money management, Job skills and planning for the future. Clients will engage in an experiential training that gives them hands on application for real life experience so they can feel confident in utilizing new learned skills in real life situations. Life skills also utilizes community partners such as bankers, business men, education and employment experts to help teach the clients and also gives these agencies and individuals an opportunity to get involved in their community and learn more about the populations we serve. Life skills training curriculums will be evidence based and constantly reviewed to update outdated materials and to promote a positive, quality experience for the clients. Life skills facilitators will hold a bachelor’s degree in a social service field and have a passion for the populations we serve. All facilitators will trained on the curriculum, group dynamics and facilitation skills. New Beginnings will also utilize master level social service students as co-facilitators for the life skills training. Education services-
Many times, a barrier for success for our clients is education. Education services can be learning to read, obtaining a high school diploma or GED, enrolling in secondary education or needing a tutor or mentor to help them reach their goals. New Beginnings Hopes and Dreams will help our clients with their education needs through one-on-one assistance with education enrollment, financial aid applications, advocacy to address barriers to their success in meeting their education needs, working with community partners such as local School districts and local colleges and Universities, to technical schools, and other crucial education partners to help the clients meet their education needs and goals. Other services offered will be education training, GED prep classes, SAT prep classes, and individual education assessments. The education specialist will have a bachelor’s degree in a social service or education field and experience within the education system. Clients will meet with their education specialist monthly or as needed. Employment services-
Another barrier that many youth and families face in become productive independent citizens of the community is a lack of employable job skills and experience. New Beginnings Hopes and Dreams will work with youth and families to develop crucial skills need for employment and will help clients secure jobs with a livable income. New Beginnings Hopes and Dreams will also partner with local business and community experts for trainings, job shadowing opportunities and employment. Circles of support/ family group conferencing- Circle of Support (COS)
A circle of support is a strength based model that builds on the positive qualities of a clients and involves a group of people coming together to help formulate, promote and support the goals of a person in need. The client is heavily involved in the circle of support in order to empower them to take ownership of the decisions in their lives. It is also based on the understanding of the importance of relationships in our lives. A circle of support brings together individual who have a relationship with the client, a person who has an invested interest in their success or a person in the community who can offer a resource to help the client meet their goals. This model gives the client a sense of belonging to a community, a knowledge that others are invested in their success and a realization that others truly care about them and want to help them reach their goals. The nature of the people who participate in the circle, how often they meet, the focus of the meetings involve the client’s feedback and needs. Participants and the client take on tasks to support the client and action plans are develop with a specific timeline for accomplish the assigned tasks. At re-conferences, the plan is reviewed, discussed, modified and if needed new tasks are developed with a new action plan. They can also change over time for any single circle as circumstances change. Family Group Conferencing (FGC)
Family group conferencing is very similar to a Circle of Support but involves the family system instead of an individual. It brings parents, extended family, friends and acquaintances into the decision making process in a way usually reserved for professionals. The family group conferencing allows the family to be the decision makers. The importance of the family’s involvement in the process is reinforced at every step from the referral, to planning and the meeting itself. Participation is voluntary. The family group conferencing model is a family centered process that allows families to be the driver of their own destination and taps in to the unique culture,
New Beginnings Hopes and Dreams will offer clients the opportunity to engage in a Circle of Support or Family Group conferencing. Facilitators will have a bachelor’s degree in social service field and be trained and certified in the COS and FGC.
