JaxCareConnect is a joint collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents in finding a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to health care in order to support best possible health outcomes. Navigating health care can be an o
verwhelming process. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a health care provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their health care journey. JaxCareConnect aims to better inform uninsured neighbors about their health care options so that all Duval County residents have access to comprehensive primary care—regardless of insurance status or ability to pay. JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.
