🩺 Connecting uninsured residents of Duval County to no-cost and low-cost Primary Care.

JaxCareConnect is a joint collaboration created by the local nonprofit safety net of free and charitable clinics to assist uninsured adult Duval County residents in finding a primary care medical home. In addition to establishing primary care, we evaluate the whole person and explore other barriers to health care in order to support best possible health outcomes. Navigating health care can be an o

verwhelming process. The JaxCareConnect team is here to compassionately serve our neighbors and provide guidance about access to a health care provider. Our Patient Health Advocates work individually with each client to connect them to a primary care doctor, navigate community resources, and provide ongoing support along their health care journey. JaxCareConnect aims to better inform uninsured neighbors about their health care options so that all Duval County residents have access to comprehensive primary care—regardless of insurance status or ability to pay. JaxCareConnect was created in collaboration with Agape Community Health Center, Community Health Outreach, Muslim American Social Services, Mission House, Sulzbacher, Volunteers in Medicine and We Care Jacksonville.

We are grateful for opportunities to come together and give back alongside incredible community partners. 💚

Recently, our team spent the day volunteering with Rethreaded, helping deconstruct donated leather airplane seats that will be transformed into beautiful products crafted by survivors of human trafficking. ✂️👜

It was a meaningful reminder that every task, no matter how small it may seem, can play a role in creating opportunity, dignity, and hope for others.

🚍 Join us for a special edition of Third Wednesday at The Church of Oakland on June 17!

In addition to our regular community resources and doctor’s office on wheels, we’re excited to welcome the Sulzbacher Social Services Bus from 9 a.m. to 12 p.m. Representatives from Sulzbacher and Changing Homelessness will be onsite to assist with housing resources, SSI/SSDI applications, and other supportive services.

👂 Jacksonville Speech & Hearing Center will also be providing free hearing screenings from 9 a.m. to 2 p.m.

Join JaxCareConnect on Thursday, July 9th as we canvas the OutEast community and help spread awareness about local healthcare resources and services. 💚

We’re looking for 10–15 volunteers to help distribute doorhangers. No experience needed, just comfortable walking outdoors and engaging with the community! 🚶🏽‍♀️🚶🏾‍♂️

We were honored to attend 's Medical Mission at Home event this past month. 💚

Through the incredible collaboration of community partners, volunteers, and healthcare professionals, 224 neighbors facing barriers to care received vital health services, resources, and support.
