The SBDF is supported by the University of Iowa Hospital and Clinics - Foundation. Shelly Warner Bridgewater succumbed to the devastation of Preeclampsia and HELLP syndrome in 2005 after delivering a premature - but healthy baby girl. New Research Regarding Women with Mild Chronic Hypertension in Pregnancy in the United States

We have had some questions about this recently, so we are reposting the information we shared in April, 2022.

Some of you have seen your doctors or the modmins reference new recommendations to start BP meds for US women with chronic hypertension at 140/90, instead of at severe range of 160/105-110, and may be wondering what this means.

In April, 2022, several well-known researchers in the field of preeclampsia published a paper in the New England Journal of Medicine, regarding the Chronic Hypertension and Pregnancy (CHAP) Trial. This trial studied over 2400 women at multiple centers, randomizing the women to BP treatment at either 140/90 or 160/105.

The 2019 ACOG Practice Bulletin on chronic hypertension previously recommended waiting to start BP meds until around 160/105-110 for women with chronic hypertension because of concerns about fetal growth restriction. The CHAP Trial found that starting BP meds at 140/90 compared to 160/105 led to significant reductions in preeclampsia and preterm birth, with no increase in fetal growth restriction.

In light of this information, ACOG and SMFM have issued statements supporting the use of BP meds at 140/90 in women with chronic hypertension during pregnancy, rather than waiting for severe range. (This is similar to established recommendations in several other countries other than the US.) While a new practice bulletin has not yet been issued, some doctors are already using a threshold of 140/90, so some of you may see a change from a previous pregnancy.

Researchers at the University of Iowa are trying to learn more about the hospital experience of people who had preeclampsia, eclampsia, or HELLP. regardless of where you live.
