eive Service:
1) Must live in the above zip codes.
2) Gather these items.
- Photo ID of all adults.
- Social Security cards of all household members.
- Current pay stub/proof of income
- Disability if applicable.
- Food stamps if applicable.
- Proof of address. (Examples: Any mail that was mailed to household with name on it like utility bill, or letter.)
3) Bring paperwork to Fairmount Community Outreach and our staff will get you started.
