Place: Methodist Church Family Life Center

** must be the legal guardian of the child
** must bring birth certificate for each child, proof of income, & proof of residency**

***please do not bring your children to this appointment***

The date for Angel Tree Applications to be filled out is TUESDAY, OCTOBER 8,2024 at the First Methodist Church Family Life Center from 2:00-5:30pm.
THIS WILL BE THE ONLY DAY!

Things you MUST have in order to fill out an application:

•Birth Certificate for each child you are filling out an application for

•valid ID

•2 proofs of income

•proof of residency (must live in Chickasaw county)

***YOU MUST BE THE LEGAL GUARDIAN OF THE CHILD/CHILDREN YOU ARE FILLING OUT AN APPLICATION FOR***

We will be filling out applications for Project Noel/Angel Tree on Thursday, September 21,2023 from 3pm-6pm at The Methodist Church Family Life Center. In order to fill out an application, you MUST live in Chickasaw County.

**THIS WILL BE THE ONLY DAY WE WILL TAKE APPLICATIONS**

You must bring:

•Birth Certificate for each child you are filling out an application for & you must be the legal guardian of the child/children.

•a valid drivers license or identification card

•proof of income

•2 proofs of residency
(light bill, water bill, etc.)

We will also be filling out applications for Golden Angels for food boxes.

You will need to bring a valid identification.

Thank you to all who made donations & adopted families for the Angel Tree this year! It was a HUGE SUCCESS!! Also, thanks to so much to the HHS Anchor Club for helping organize the gifts & putting boxes together & to the HHS Basball team for helping distribute gifts & food boxes!
