Doctors, hospitals, and other health care providers care for your illnesses and injuries, but now they are also placing more emphasis on helping you stay well before you even get sick or hurt. To do that, health care providers - hospitals, doctors, labs, nutritionists and others - and social service providers of all types - commit to work together to meet your every need in keeping you well or get

ting you well. That's why programs and services that focus on prevention and wellness are so important and necessary for your total health. You may have heard terms like "population health" and "coordinated health care." These are simply ways in which your health and the health of your family, neighbors, and larger community can be managed. LIHC is a working group of hospital members, local health department personnel, representatives from social service organizations, public health specialists from colleges, and others who form the core of health and human services for all Long Islanders. LIHC's goal is to help Long Islanders understand why prevention and primary care are important, why each patient must play a more active role in their own health, what resources and services are available here on Long Island, and how these resources and services can be accessed. You can use the links throughout our website to connect you to the care and services you need.
