As a requirement of Health Reform, non-profit hospitals are required to complete and act on a Community Health Needs Assessment (CHNA) every three years to maintain their 501 (C) (3) status. Healthy coalition was formed in April 2012 as a result of the Community Health Needs Assessment, which found obesity prevention as one of the community health needs in Barry County. This coalition was formed through a partnership between Pennock Health Services and the Barry-Eaton District Health Department and involves community participation from a variety of supporters. The coalition consists of representatives from business, government, schools, medical community, nonprofits and social service agencies. Healthy coalition is to “reduce the obesity rate among Barry County residents to be at or lower than the State of Michigan average according to the County Health Rankings.”
The B. Healthy coalition has met monthly since April 2012 to assess the factors that lead to obesity in Barry County, as well as develop a shared vision of a community that reduces obesity. The coalition collaboratively plans for interventions using a broad set of evidence-based strategies that take into consideration health disparities related to obesity in Barry County assuring a multi-faceted and comprehensive approach to obesity prevention for the entire community.
