Qualifications:
• Youth being referred must reside in, attend or have attended one of the following Michigan school districts: Haslett, Okemos, East Lansing, Mason and or Holt.
• Youth must be between the ages of birth to 21 years.
• A verification form must be completed and signed by a licensed, treating physician and must clearly indicate the life threatening and/or severely life altering medical condition of the youth being referred.

Request for Assistance
• Consideration is made without regard to gender, race, ethnic origin or religious preference.
• Although we cannot meet every need, we strive to provide assistance to each youth who applies.
• All information is held in strict confidence.
• Applications will be reviewed at the next scheduled Board meeting.
• All decisions are final.
• Any funding will be paid directly to creditors, not the applicant.
