Minority AIDS Initiative Keeping HIV positive individuals connected to care by eliminating any barriers that they may encounter.

MAI is a DPH funded program that offers a reconnect or connect to care services using the referral process. Providers should submit referrals along with updated releases and consents. MAI then can seek out a client who has missed 2 or more appointments with Case Management and or has outdated lab work over 6 months or missed Ambulatory Care appointments. MAI staff will screen each individual to en

sure these three components; ID care, MCM, and CADAP/CIPA are in place which can be an added benefit for Case Managers whom a client is being (re) connected to. It is the MAI goal to refer clients to all agencies and resources in the community in an effort to ensure that clients are receiving care on all levels but also increase the opportunity for a client to remain connected to care which ultimately can help to prevent the spread of HIV.
