If you've been approved for a TotalAssist grant and are uncertain about the diagnosis verification process, don't worry—we're here to guide you through the next steps.

📠 We fax a patient-specific Diagnosis Verification Form directly to your healthcare provider for them to complete and return to us. If they don't receive it or need another copy, simply reach out to us via our Contact Us form and verify their fax number: https://bit.ly/4wQjf2s

✅ Once your provider submits your Diagnosis Verification Form, our team will review and approve it. During this brief review period, your portal account may still indicate your diagnosis verification is missing but there's no need to worry, we have your documents and the required actions section of your portal will update soon.

Plus, you can still submit claims for covered expenses during this verification period. As long as your provider submits the form within 30 days of your approval, your grant remains active.
