-Transition Planning
TPS will coordinate sets of activities with students and their family that will focus on improving the academic and functional achievement to promote the student’s movement from school to post-school activities. Post-school activities can include college, vocational training, employment, continuing and adult education, adult services, independent living or community participa
tion.
-Training and Consulting
TPS will provide training to staff, families, community partners and self-advocates on a variety of topics including the areas of rights, person centered transition planning, communication, informed decision- making, consent and group facilitation and other service related topics.
-Provider Support
TPS will provide consultation services to new and existing Provider Agencies certified by Ohio DODD which includes but not limited to:
Pre-compliance Review BEFORE the State review
Program Development needs (i.e. Human Resources services and support
Ongoing staff development training (i.e. Incidents Adversely Affecting Health and Safety/Major Unusual Incident-MUI)
-Supported Employment
TPS will provide ongoing supports that will enable participants, for whom competitive employment is unlikely absent the provisions of supports, and who because of their disabilities need supports, to perform in a regular work setting.
-Vocational Habilitation
TPS will provide vocational services needed to assure the optimal functioning of individuals. Vocational services designed to teach and reinforce habilitation concepts related to work including responsibility, attendance, task completion, problems solving, social interaction, motor skill development and safety.
-Transportation
TPS will provide transportation to and from day service. TPS will provide transportation for purposes of shopping, banking, social events, medical appointments, getting to and from work and similar activities for people with disabilities that need special accommodations and are unable to utilize other available means of transportation.
-Advocacy and Referral-Free Service
TPS will provide advocacy and referral services to individuals with and without developmental disabilities and their families.
